Which insurance plans are you in-network with?
Balanced Baby Lactation is a private-pay practice designed to provide individualized, in home lactation and postpartum support. Payment is due at the time of service. A superbill can be requested for possible out-of-network reimbursement depending on your insurance plan and visit type. A portion of your 4th Trimester Support Package may be eligible for Insurance reimbursement.
Using Insurance Benefits: Some families may be eligible for reimbursement through:
- Out-of-network lactation benefits
- HSA/FSA Accounts
- Employer reimbursement programs
Coverage varies by plan and out of network reimbursement is not guaranteed.
Do you provide services in both Illinois and Wisconsin?
Yes. Balanced Baby Lactation serves families in both Illinois and Wisconsin.
Available services may vary based on your location and state specific regulations. In home and virtual lactation support are available throughout our service area and recommendations regarding available services will be discussed during the consultation request process.

Are payment plans available for fourth trimester support packages?
Flexible payment plans are available for families enrolled in the Balanced BeginningsTM Fourth Trimester Signature Support Packages.
12 Week Signature Plan Payment Plan Option $1875
- $ 100 due at enrollment or prenatal visit
- $ 525 due at first in-home visit
- $ 625 due at 4-6 week visit
- $ 625 due at 6-8 week visit
8 Week Complete Package Payment Option $1250
- $100 due at enrollment or prenatal visit
- $525 due at first in-home visit
- $625 due at time of 4-6 weeks
4 Week Foundation Package Payment Option $725
- $100 due at enrollment or prenatal visit
- $625 due at first in-home visit
Pay in full option available at enrollment.
A valid card on file is required for all payment plans.
Applicable travel fees for in-home visits are billed separately at the time of services.
Are single visit and follow up services available?
In-Home Lactation & Feeding Consultation*
- $350 initial consultation
- $250 follow-up consultation
Telehealth Lactation & Feeding Consultation
- $225 initial consultation
- $175 follow-up consultation

What are the home visit/travel convenience fees?
A $50 convenience fee applies to in-home visits within 20 miles of Twin Lakes, Wisconsin.
For visits located more than 20 miles away, additional travel fees may apply based on distance and scheduling availability.
Fourth Trimester Support Package Families
Travel within 20 miles of Twin Lakes, Wisconsin is included for families enrolled in the Fourth Trimester Support Packages.
For visits located 20-40 miles away, a $50 travel fee per in-home visit applies. Travel beyond 40 miles may be accommodated on a case-by-case basis and may require an additional travel fee.

My baby has already arrived – can we still join the Fourth Trimester Support Package?
You and your baby may begin this support program at any point during the first 12 weeks postpartum. Care is individualized to your family’s goals and can be tailored to meet your needs wherever you are in your feeding and postpartum journey. Please call to discuss what package will work best for your family and situation.
Can the Fourth Trimester Support Package support NICU graduates or medically complex feeding journeys?
Absolutely. The program can be individualized to support NICU graduates, feeding transitions, pumping journeys, oral feeding development, and complex infant feeding needs after a discharge. Care is tailored to your baby’s unique medical and developmental needs while supporting your family’s feeding goals during this transition period.
What if I’m combination feeding or exclusively pumping?
Balanced Baby Lactation services and Balanced BeginningsTM Support Packages are designed to support families in meeting their feeding goals – whether that includes breastfeeding, pumping, bottle feeding, supplementation, formula feeding, or combination feeding.
Every family’s journey looks different, and feeding plans may evolve throughout the postpartum period. My role is to provide evidence-based guidance, individualized support, and compassionate care without judgement, while helping you feel confident in how you nourish and care for your baby.
Support may include:
- breastfeeding and latch support if desired
- pumping guidance
- bottle introduction and bottle refusal support
- supplementation planning
- low milk supply support
- return-to-work feeding plans
- feeding transitions as your baby grows
Does this replace my pediatrician or well child visit?
No. Balanced Baby Lactation services are designed to complement – not replace- your baby’s pediatrician, primary care provider, obstetric provider, or emergency medical care.
Families should continue all recommended newborn pediatric visits, vaccinations, screenings, and routine medical care with their established healthcare providers. Balanced Baby Lactation does not function as your child’s medical home or routine primary care practice.
The Balanced BeginningsTM Fourth Trimester Support Packages are intended to provide personalized feeding support, newborn guidance, developmental education, and postpartum support during the transition into early parenthood while integrating medical expertise when clinically appropriate.
If concerns arise that require ongoing medical management, diagnosis, emergency evaluation, or specialty care, appropriate referral back to your pediatrician or healthcare provider will be recommended.
No Show, Cancellation & Refund Policy
Single consultation visits require reserved appointment time and individualized preparation. Missed appointments, late cancellation, convenience fees, and completed consultation services are non-refundable.
Families who need to reschedule are encouraged to provide as much notice as possible. Rescheduled appointments are subject to availability. Late cancellations or repeated missed visits may require rescheduling based on availability. Cancellations of less than 24 hours will incur a $50 late cancellation fee.
If a provider arrives for a scheduled in-home visit and the family is unavailable or unreachable, a missed appointment fee of $100 may be charged to the card on file.
Packed-based services are subject to separate agreement terms and policies.
Are Your Services Insured and Professionally Licensed?
Balanced Baby Lactation is fully insured and operated by a licensed healthcare professional. Services are provided within the scope of practice applicable to your location and visit type. Jenell Jackley is a licensed Pediatric Nurse Practitioner and International Board Certified Lactation Consultant (IBCLC), carrying professional liability insurance for the services provided.
For families receiving medical lactation care in Illinois, services are provided under Illinois APRN Full Practice Authority. For Wisconsin families, services are provided in accordance with current Wisconsin licensure and scope of practice requirements.
Good Faith Estimates and Promotional Credits
How do payment and Good Faith Estimates work?
Balanced Baby Lactation is a self-pay practice. Payment is due at the time of service. If you are uninsured or choosing not to use insurance, you have the right to receive a Good Faith Estimate of the expected cost of your care under the No Surprises Act. A written Good Faith Estimate is available upon request and will be provided as required by federal law. Eligible services may also qualify for out-of-network insurance reimbursement through a superbill.

Your Right to Receive a Good Faith Estimate
Under the No Surprises Act, health care providers are required to provide uninsured or self-pay patients with a Good Faith Estimate of the expected charges for medical services.
At Balanced Baby Lactation, we believe in transparent pricing. Our service fees are available on our website, and you have the right to receive a written Good Faith Estimate before receiving non-emergency services.
A Good Faith Estimate:
- Explains the expected cost of your scheduled services.
- Does not include unexpected medical needs that may arise during care.
- Helps you understand your anticipated financial responsibility before treatment.
If you receive a bill that is substantially higher than your Good Faith Estimate, you may have the right to dispute the charges under federal law.
You may request a Good Faith Estimate at any time before scheduling or receiving services.

Promotional Package Credit
Promotional package credits are available only through select marketing campaigns and may be applied toward the initial home visit included in an eligible Balanced Baby package or initial in home visit. Travel fees will apply as outlined in travel home visit policy. The promotional credit is deducted from the first visit only and is not applied across subsequent visits or other package components. Only one promotional credit per visit.
Package Reservations & Payment Policy
Due to the personalized and ongoing nature of the Balanced BeginningsTM Fourth Trimester Support Package, space is intentionally limited to ensure dedicated support for enrolled families.
A signed package agreement and payment method on file are required prior to the start of services.
Payment plans are available for family convenience; however enrollment reserves dedicated availability and support time within the practice.
Services already provided – including consultations, messaging support, care planning, and completed visits – are non-refundable.
If a family chooses to discontinue the program early, any remaining unused portion of services may be reviewed on a case-by-case basis. Refunds are not guaranteed and may be adjusted based on services already rendered and administrative reservation costs.
